In 1982, the American College of Nurse Midwives (ACNM) created a Special Projects Section (SPS) whose goal was to combat deteriorating maternal and infant health, improve the lives of women, and advance the field of midwifery. By the late 1980s, the SPS adopted an increasingly international focus with notable Safe Motherhood projects in Nigeria, Ghana, Uganda, and Kenya aimed at combating maternal deaths, promoting family planning services, and strengthening midwifery programs. At the backdrop of these international projects was a maternal healthcare crisis in the US and a sustained rift between nurse-midwives and the American College of Obstetricians and Gynecologists (ACOG) over the legality, scope, and practice of midwifery in America. Drawing from multiple sources, notably the NLM’s extensive collection on the American College of Nurse-Midwives, I examine the ACNM’s maternal and reproductive health advocacy work in Nigeria during the 1980s and 1990s while fighting for autonomy, recognition, and better maternal welfare policies at home. </video> <video> Prefrontal Lobotomy in the Treatment of Mental Disorders (GWU, 1942) Description: This film describes and demonstrates a prefrontal lobotomy, an operative procedure employed in mental disorders resistive to other methods of treatment. The procedure consists of cutting the white matter in each frontal lobe in the plane of the coronal suture. This passes just anterior to the frontal horn of the ventricle and interrupts the anterior thalamic radiation. This film includes a written description of the procedure, review of landmarks on the skull and frontal lobe on a demonstration skull and brain, operation on a live patient, and X-rays taken after the operation. However, the cost of conducting these trials can be prohibitive. In addition, evidence from the trials frequently rests on narrow patient-inclusion criteria and thus may not generalize well to real clinical situations. Given the increasing availability of comprehensive clinical data in electronic health records (EHRs), some health system leaders are now advocating for a shift away from traditional trials and toward large-scale retrospective studies, which can use practice-based evidence that is generated as a by-product of clinical processes. Other thought leaders in clinical research suggest that EHRs should be used to lower the cost of trials by integrating point-of-care randomization and data capture into clinical processes. We believe that a successful learning health care system will require both approaches, and we suggest a model that resolves this escalating tension: a “green button” function within EHRs to help clinicians leverage aggregate patient data for decision making at the point of care. Giving clinicians such a tool would support patient care decisions in the absence of gold-standard evidence and would help prioritize clinical questions for which EHR-enabled randomization should be carried out. The privacy rule in the Health Insurance Portability and Accountability Act (HIPAA) of 1996 may require revision to support this novel use of patient data. About the speaker: Dr. Manisha Desai is a Professor of Medicine, Professor of Biomedical Data Science, Professor of Epidemiology and Population Health, and Associate Dean of Research at Stanford University School of Medicine. She also serves as the founding Director of the Quantitative Sciences Unit, a data science unit consisting of faculty and staff who practice data science to address critical biomedical questions at Stanford. She is also the Principal Investigator of a Data Coordinating Center for a large multi-center trial investigating a therapy for the hospitalized pneumonia patient (ARREST Trial) and of an R01 to develop new methods for the analysis of high dimensional accelerometer data. Dr. Desai also served as Principal Investigator of the Data Coordinating Center for the Apple Heart Study, in which the goal was to characterize performance of an app to identify atrial fibrillation in the general population. Prior to joining Stanford, she was on the faculty of the Department of Biostatistics at Columbia University. Dr. Desai received her PhD in Biostatistics from the University of Washington. Prior to the presentation of each case, a brief written description of the patient's pertinent medical history, symptoms, clinical course, and treatment is shown. Dog bites appeared to be responsible for the rabies in all four cases. The four cases differed in the length of the incubation period and in the severity of the symptoms affecting the central nervous system.
