Description: In my experience, about 5% of patients receiving the ClearView 3 multifocal lens are dissatisfied with their reading vision. These patients see shadows, secondary images, or blurred vision when reading text or images on their phone or in print. The far vision in these patients tends to be excellent. This 5% group of ClearView patients is typically bothered enough to want additional surgery to improve their near vision. We have been using the Odyssey multifocal lens as the lens implant replacement when we remove the ClearView 3 lens in a procedure called intraocular lens exchange. For the patients that have received the odyssey Lens after having the clearview 3 lens removed, all have been satisfied with their quality and range of vision far/mid and near. We interview a patient who was unhappy with her clearview 3 lenses. She describes her vision before and after receiving the clearview 3 lens, and then describes her vision with the odyssey lenses. Shannon Wong, MD Description: I have been using the Bausch and Lomb LI61AO monofocal lens implant for more than 20 years in nearly 20,000 patients. Like all monofocal lenses, the bausch and lomb LI61AO lens delivers great vision for patients who don't mind wearing glasses full-time to see their best. Medical insurance typically covers the cost of monofocal lenses. The LI61AO is my preferred monofocal because of the following characteristics: 1 Lowest rate of dysphotopsias 2 Bag or sulcus placement 3 Insensitive to decentration due to the aspheric design 4 Can be used for intrascleral haptic fixation in cases where placement in the sulcus or the bag is not possible 5 Ease of insertion 6 Outstanding quality of vision day and night Financial disclosure: I have no financial interest in the products discussed in this video. </video> <video> Intraocular lens exchange 4 years after initial surgery. Switching from monofocal to multifocal lens Description: This video highlights 3 points about cataract and premium lens surgery: (Financial disclosure: I have no financial interest in the products discussed in this video). 1 Do patients ever have a monofocal lens inserted, then come to realize that they want full-range of focus to minimize their reliance on glasses and then choose to have their monfocal implant removed and replaced with a multifocal implant? 2 When does it become impossible to remove one implant and have a different lens implant inserted? 3 Why don’t ophthalmologists recommend intraocular lens exchange more often for patients? a. Most patients are happy with their original lens implant b. Most ophthalmologists do not recommend intraocular lens exchange unless it is deemed medically necessary and safe. c. We are already busy enough helping people who want to have cataract or premium lens replacement surgery. If we were to get involved with offering lens exchanges routinely, then there simply would not be enough time available. d. Upgrading from one lens implant to another implant is often considered elective or not medically necessary, the process is complex and patients have to be willing to pay out-of-pocket for elective intraocular lens exchange. e. Most ophthalmologists do not perform elective intraocular lens exchange because the process is very time consuming, the surgery can be complicated and the patients can be more demanding than usual and may have unrealistic expectations. CONCLUSIONS: 1 It is possible to remove and replace lens implants years after they were inserted and insert lens implants today that were not available in past years. 2 When does it become impossible to perform intraocular lens exchange? It can be done pretty much indefinitely if you and your surgeon have a long discussion about the risks and benefits of performing intraocular lens exchange and if you and your surgeon are aligned with regard to realistic expectations about the surgery and the outcomes of repeat surgery. Additionally, both parties have to agree that most of the time, intraocular lens exchange will be considered elective surgery, not covered by insurance and the patient will be fully responsible for the cost of the surgery. 3 Most ophthalmologists have great and valid reasons for recommending against elective intraocular lens exchange surgery. We are highly selective in performing this type of surgery but do offer it for patients that we believe to be appropriate candidates for this type of surgery. </video> <video> Subluxed synergy multifocal lens. Thank you for watching! </video> <video> Eye surgery with oral valium sedation. No IV. No anesthesiologist. How do patients actually feel? Description: Since 2000, I’ve been performing all eye surgeries in our surgical center with oral sedation using 10 mg of valium . The cost of this 10 mg valium is less than a nickel per pill. We don’t use IV sedation. There is no anesthesiologist or nurse anesthetist in the room. Let me assure you that the patient experience for eye surgery in our surgical center is almost always positive. Patients having cataract or lens replacement surgery while awake on valium sedation sail through the process and the experience is uniformly positive. We asked 25 of our patients that had cataract or lens replacement surgery in our ASC the following 3 questions: BEFORE SURGERY On a scale of 0-10 where 10 is you are are super nervous/stressed and 0 is you are not stressed at all, how nervous are you about having cataract or lens replacement surgery (before your surgery)? AFTER SURGERY On a scale of 0-10 where 10 is your are super nervous/stressed and 0 is you are not stressed at all, how stressful was your surgery? On a scale of 0-10 where 10 is you experienced a lot of pain during surgery and zero is you experienced no pain at all, how painful was your surgery? The average stress score before surgery was 3.24/10. The average stress score during/after surgery was 1.6/10. The average pain score during surgery was 0.78/10. Conclusions: So, fear not. The patient experience for eye surgery with valium is consistently positive. We wouldn’t have been doing eye surgery in this method for this long if our patients’ experience wasn’t consistently great. Hopefully this video gives you some assurance that your eye surgery will be comfortable and your experience pleasant.
