Secretion in the major airways such as the trachea and the main bronchi are usually coughed out or can be effective removed by suctioning. However, air passage in the peripheral lung segments required postural assistance to drain their secretion into the main airways, because the patient usually sits in an upright position, secretions are likely to accumulate in the lower parts of lungs. When the postural drainage is used, the patient is placed sequentially in different positions, so that the force of gravity helps to drain secretions from the smaller bronchial airway to the main bronchi and trachea. Assessment of the area involved is determine by auscultation, percussion and by x-rays.
